Summary Coordinates the clinical management of the transplant patient including the transplant evaluation, the transplant process and follow‑up care. Maintains strong, collaborative relationships with the medical director of the organ transplant program, transplant surgeons, and referring faculty members. Serves in an on‑call capacity for the transplant program.
Key Responsibilities
Participates in coordination of organ recipient and donor evaluation, including scheduling diagnostic studies and blood and tissue typing.
Acts as a liaison among all transplant team members regarding candidate evaluation, status of candidacy, and transplant activity.
Maintains candidate waiting list and coordinates the notification plan; serves as a liaison to affiliated agencies (MIOP, UNOS) to ensure the waiting list is current.
Develops and maintains a computerized database to track patient status and program outcomes for internal and external reporting purposes.
Coordinates the care of inpatients on the transplant surgical service by collaborating closely with the staff of the transplant unit and the transplant team; participates in process–action teams to improve patient care when appropriate.
Assists physicians in the daily evaluation and management of all patients admitted to the transplant service.
Coordinates ongoing outpatient follow‑up care through the transplant clinic and referring physicians.
Records pertinent patient data and notes patient progress in the medical record; performs review function for the appropriateness of admission and continued hospitalization.
Participates in interdisciplinary team meetings to provide a comprehensive plan of action/care for transplant patients після.
Serves as a liest to regulatory agencies to monitor and ensure compliance with established standards of care.
Collaborates with the nursing staff of the transplant unit to provide patient/family education pre‑ and post‑transplantation.
Identifies patient and family educational needs and develops educational programs and materials accordingly.
Serves as a consultant for staff and patient education for all hospital departments providing care to transplant patients.
Provides on‑call responsibilities related to coordination of organ procurement and transplantation activities.
Qualifications
Bachelor's degree in Nursing (BSN). A certification or Master of Science Degree in a related area is preferred.
Three years of pediatric nursing experience; experience in a pediatric ICU and/or with solid organ transplant is strongly preferred.
Licensed to practice as a Registered Nurse in the State of Michigan.
