SUPPORT NURSE – LPN – SUMMERSTONE HEALTH AND REHAB CENTER
Liberty Healthcare and Rehabilitation Services seeks an experienced
Support Nurse – LPN
to provide quality nursing care in a compassionate environment. This role supports nursing personnel, assists in patient care, and ensures compliance with regulations.
Responsibilities
Assist nursing personnel (charge nurse, Director of Nursing) on a specified unit during assigned work hours.
Respond to calls for staffing coverage as required.
Provide resident care in accordance with the Nursing Practice Act under RN supervision.
Delegate duties to non‑licensed nursing personnel and monitor performance; help prepare work schedules.
Collect quality assurance data and support corrective action implementation.
Assist with infection control and maintain unit cleanliness.
Maintain accurate records, orders, and assessments for new admissions.
Complete resident assessments per MDS Nurse or Director of Nursing.
Attend Nurse Department meetings on schedule.
Monitor and measure decubitus ulcers weekly and complete integument watch assessment forms.
Report performance issues to the RN for disciplinary action, including documentation.
Assist with lab requests and re‑posting lab work, reviewing results, and notifying attending MD.
Respond to emergencies, perform CPR when needed.
Notify attending MD of patient changes, illness or death; implement orders as instructed.
Support general duties: rounds with doctors, reviewing and updating orders, verifying nursing summaries.
Provide hall coverage as needed.
Coordinate the health care team, including rehab aide duties.
Perform other duties assigned by Director of Nursing per NC Board Of Nursing Practice Acts.
Qualifications
High school graduate.
Licensed Practical Nurse (LPN) with an accredited nursing school diploma, current NC LPN license and proof of renewal.
Experience in geriatric nursing and ability to provide quality nursing care following doctors' orders.
CPR certification maintained yearly and attendance at in‑service training and staff meetings.
Decision‑making skills in nursing and unit situations; ability to recognize errors and ensure patient safety.
Ability to work well under pressure, problem solve, and handle varied tasks.
Familiarity with personnel, department, and facility policies and local, state, and federal regulations.
Proficiency in maneuvering medication carts, treatment carts, and nursing equipment.
Background checks and a drug‑free workplace are required.
