Role Overview Legacy Care is seeking a skilled and compassionate Nurse Practitioner to join the team on a PRN (as‑needed) basis in Williamsburg, VA. The successful clinician will deliver comprehensive primary and acute medical care to residents in post‑acute and long‑term care facilities. This PRN role requires flexibility to meet variable scheduling needs, strong clinical judgement, and a commitment to person‑centred, evidence‑based practice. Core responsibilities include resident assessment, diagnosis, treatment, medication management, interdisciplinary collaboration and participation in quality improvement activities.

Key Responsibilities — Nurse Practitioner, Post‑Acute Care
Conduct comprehensive assessments and develop individualized care plans for residents in post‑acute and long‑term care settings
Provide diagnosis, treatment, medication management and appropriate follow‑up consistent with current evidence‑based practice
Respond promptly to acute changes in condition, initiate timely interventions and coordinate transfers of care when necessary
Collaborate with nursing staff, physicians, therapists, social workers and facility leadership to deliver holistic, person‑centred care
Participate in facility rounds, care‑planning meetings and interdisciplinary case conferences
Document clinical encounters accurately and promptly in the Electronic Medical Record (EMR)
Support medication safety initiatives, including antimicrobial and antipsychotic stewardship
Provide clinical consultation and education to facility staff and medical colleagues on complex clinical issues and best practices
Maintain professional standards of practice, including infection control, documentation and regulatory compliance
Essential Qualifications And Skills — Nurse Practitioner, Williamsburg VA
Master’s degree in nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited institution
Active and unrestricted Virginia APRN and nurse practitioner licensure
Prescriptive authority as required by Virginia state regulations

Clinical experience in post‑acute care, long‑term care, geriatrics or related settings preferred
