Full Time Monday-Friday 8AM-5PM

As an RN Case Manager, You Will:
Assess and monitor physical, emotional, and psychological needs of patients.
Create hospice care plans that align with the patient's wishes and goals.
Direct nursing care: administer medications, treatments, and interventions.
Provide pain and symptom management.
Educate and support the patient's family and caregivers.
Collaborate with interdisciplinary team.
Maintain accurate and timely documentation.
Participate in on-call rotation as required by local branch.

Qualifications Required:
Graduate of an accredited nursing school with current licensure to practice in state of operation.
One year nursing experience.
Preferred:
3+ years experience.
Experience in hospice or a similar setting preferred.
Knowledge of the hospice philosophy of care.
Commitment to clinical and documentation excellence.
Must be a licensed driver with an insured automobile in good working order.
