$50,000.00/yr – $60,000.00/yr Position Summary

The Housing Case Manager provides comprehensive, people-centered support to individuals in recovery from substance use and/or mental health challenges. This position combines traditional case management with direct, hands‑on guidance in the community. The Case Manager helps clients develop essential life skills, connect with resources, and build confidence and independence as they progress through recovery. This is an active, community‑based role. The Case Manager works alongside clients in real world settings, offering mentorship and skill‑building in areas such as financial literacy, grocery shopping, healthy living, transportation, and community integration. Key Responsibilities

Provide case management services for clients engaged in substance use treatment programs, including assessment, care planning, and service coordination. Develop individualized service plans based on client needs, treatment goals, and available resources. Coordinate with clinical staff, peer support specialists, and external providers to ensure continuity of care. Connect clients with community resources such as housing, employment assistance, mental health services, medical care, and legal support. Monitor client progress, track outcomes, and revise service plans as needed. Maintain timely, accurate, and confidential documentation in compliance with agency policies and state/federal regulations. Advocate for clients’ needs while promoting self-sufficiency and empowerment. Participate in interdisciplinary team meetings, case reviews, and supervision. Stay current on local resources, behavioral health best practices, and regulatory requirements. Accompany clients in the community to practice skills and access resources (e.g., grocery stores, banks, employment centers, recovery meetings). Life‑Skills Development

High school diploma or equivalent required; Associate’s or bachelor’s degree in behavioral health, Social Work, Psychology, or related field preferred. Chemical Dependency Counselor Assistant (CDCA) – Required Qualified Mental Health Specialist (QMHS III) Preferred Minimum 2-3 years of experience in behavioral health, substance use treatment, or crisis response. Strong crisis management and de‑escalation skills. Familiarity with community resources in the Cleveland, Ohio area. Ability to maintain professional boundaries and handle sensitive situations with empathy. Maintain requested billing hours. Valid driver’s license and reliable transportation required.
