Social Worker – University of Rochester
Location: 601 Elmwood Ave, Rochester, New York, United States of America, 14642.
Compensation Range $30.29 – $38.75 per hour. (Minimum and maximum compensation; annual salaries/hourly rates will be set within this range.)
Responsibilities The social worker provides clinical social work services to adult patients and their families in ambulatory clinics. Social work services include psychosocial assessments, information and referral, psychosocial treatment, and other services required to optimize the patients' health care. The social worker collaborates with the health care team in the development and implementation of the care plan of the patients and provides consultation to the health care providers regarding patients' care needs and/or community resources.
Essential Functions
Performs psychosocial assessments of patients and families referred by clinic providers or identified through high risk screening.
Provides social work services to patients with problems identified through psychosocial assessments, including crisis intervention, supportive counseling, problem solving, information and referral, respite or long‑term facility based care, and assistance with environmental problems.
Collaborates with other clinic care providers in development and implementation of interdisciplinary care plans.
Coordinates referrals to and acts as liaison to community agencies regarding services to individual patients and maintains current knowledge of community resources.
Provides appropriate assessment and intervention services for children and adults suspected of being victims of abuse, neglect, or domestic violence.
Handles risk management issues in close consultation with supervisor and Clinical Manager.
Documents social work activity in the electronic medical record.
Provides social work coverage as requested to other clinical units or programs during co‑workers' absence.
Administrative Functions
Completes required in‑service training programs and ongoing professional development activities, including those designed to enhance knowledge and skills in protective work.
Attends required Employee Health appointments within the required timeframe.
Attends all appropriate Social Work Division meetings, including but limited to division meetings, complex case discussion, transplant staff meeting, etc.
Supervises social work students (MSW or BSW).
Orients and provides ongoing peer support to new social work staff.
Other duties as assigned.
Minimum Education & Experience
MSW from an accredited social work graduate program and must obtain New York State license within eighteen months of employment.
Prior clinical experience in a medical setting and supervisory experience preferred.
Knowledge, Skills and Abilities
Familiar with medical social work and community resources.
