Job Title: Program Counselor | Program: Partial Care | Location: 725 Airport Road, Lakewood New Jersey | Position Type: Full Time | Salary: $37,132.00

Participates in providing structured partial care program services via therapeutic groups and activities; arranges services for an individual case load of consumers to optimize community resources and community linkages in collaboration with persons served.

Responsibilities

Works with persons served and the multidisciplinary team to plan and implement the plan of care and subsequent reviews reporting changes in service.

Plans and facilitates therapeutic groups and activities, which involve psycho‑education, medication education, nutrition, group counseling, behavior management, community meetings, coping skills, pre‑vocational skills, social skills, daily living skills, leisure activities, and alcohol/drug education, support groups.

Has primary responsibility for service coordination, provision or arrangement of services needed, personal advocacy, and development, review and updating of individual treatment and service plans.

Completes Intake Assessments as directed by Program Director including all of the required documentation (including, ISP, Infection control education, MSE, review fee agreements, completion of CACFP application, initial consent for treatment).

Provides timely documentation into consumer’s clinical records notating weekly participation in the therapeutic community, responses to service plan interventions, consumer advocacy and service coordination.

Engages new persons served into the program and re‑engage persons who have decreased their participation.

Actively seeks supervision to discuss pertinent clinical issues and intervention strategies to foster consistency for the persons served to progress.

Facilitates brief individual program counseling for persons served assigned to their caseload assessing each consumer for relapse, identifying consumers at risk and ensures appropriate wellness recovery action plans are implemented.

Demonstrates understanding about the impact of mental illness and/ or substance abuse and involves the support network and family of the persons served in the plan of care accordingly.

In accordance with the Department of Agriculture guidelines staff orders food from the US Food Service website: facilitates monthly menu.

Checks the temperatures of all refrigerators and ovens.

Prepare food and meals as necessary.

Competencies

Must maintain CPR/First Aid certifications.

Basic computer proficiency as required by the job.

Qualifications Education:

Bachelor’s Degree in human services field or related field.

Associate’s Degree and two years’ experience in providing human services.

Five years of human services experience or possess a relevant professional credential, such as Certified Psychiatric Rehabilitation Practitioner, bachelor’s degree in social work, certified rehabilitation counselor, certified alcohol and drug counselor, bachelor of science in nursing.

Experience:

Such designated staff members shall possess the qualifications for the primary case coordinator or counselor position and shall have training and experience in providing the specialized service.

Qualifications for the primary staff providing pre‑vocational services must include one year of experience in providing vocational evaluation, welfare to work, community rehabilitation services, transitional employment, or other work experience programs.

Qualified staff shall maintain their knowledge and expertise by continuing to receive relevant training and continuing education.
