A DAY IN THE LIFE OF A COMMUNITY LIVING SPECIALIST
In this position, you will provide person-centered care coordination with older Minnesotans and caregivers over the phone and in-person. You will be providing options counseling, advocacy, resource location assistance and education. You will research home and community-based service options and assist clients as they navigate systems and comply with specific protocols and documentation requirements.
Qualifications
2 years of experience with case management or care coordination in a long-term care, hospital, or community setting
Compliance with all state licensure requirements Bachelor's degree from an accredited college or university in a human services-related field. Social work and nursing degrees strongly preferred. Reliable transportation Heart for working with the elderly
Do you have excellent communication and interpersonal skills? Are you highly organized and detail-oriented? Do you have strong problem-solving skills? Can you successfully work independently and alongside a team? Are you great at time management? If yes, you may be perfect for this position!

This position is
hybrid,
working in the community, at home and in the office
Monday - Friday, 8:00 AM - 4:30 PM.
