What You’ll Do The Practice Transformation Coach is accountable for providing transformation support to Community Health Centers (CHCs) resulting in improved quality outcomes and network performance in Carina Health Network’s value‑based care (VBC) programs. The Coach has a defined role, working in an advisory/coaching capacity to support Carina CHCs and assist in creating and implementing individualized CHC Carina Support Plans to optimize performance. The Coach utilizes an evidence‑based approach to facilitate QI methodology and tools within CHCs, including assessments, reporting measures, and facilitating meetings with practice‑specific QI teams working toward achieving measurable outcomes.

Develop, implement, and monitor individualized CHC Carina Support Plan targeting practice transformation activitiesამც to increase success in Carina’s value‑based contracts.

Serve as a liaison, teacher, coach, and facilitator for practice transformation activities for CHCs participating in the network.

Obtain, aggregate, and analyze quality metrics and reports for care opportunities, identify opportunities to improve cost, quality, and patient experience, prioritize opportunities and address them in individualized CHC Carina Support Plan.

Participate in the development and implementation of community learning forums.

Provide onsite and telephonic CHC visit coaching using a standardized approach and demonstrate the ability to effectively lead and facilitate improvement teams toward he's measurable outcomes.

Maintain a supportive and collegial role with the primary care team and collaborate to meet provider and practice needs.

Conduct ongoing CHC assessments to learn more about practice demographics,IFUL daily operations, and current workflows.

Support the efforts of providers and practice teams on interventions resulting in cost‑of‑care savings and improved health outcomes for patients.

Provide education for practices to develop expertise with Annual Wellness Visits, metrics, and data review for quality improvement.

Work with Carina leadership on communication to physicians regarding available patient care programs through the VBC programs—both written/email communication and visits to provider meetings as necessary.

Review reports from payers and్ participate with leadership in the interpretation of the reports and actions needed.

Shadow, educate, and/or train physicians and medical staff as necessary in clinical workflows and coordination of care to improve clinical quality and achieve VBC goals. Test and pilot new VBC initiatives and work with Carina leadership on refining processes, helping achieve contractual metrics.

What We’re Looking For

Excellent computer skills required, particularly related to Microsoft applications Word, Visio, Excel, PowerPoint, and Outlook.

Ability to analyze payer quality data and navigate reports from various payer portals.

Comfort with exploring and learning to use new or unfamiliar applications or databases.

Working knowledge of health information technology and ability to connect practices to necessary resources to produce needed data/reporting.

Commitment to collaboration, professionalism, and effective communication in all interactions with physicians, CHC teams, VBC partners, and payers.

Flexible, courageous, and positive attitude about unexpected changes and fluctuating workloads.

Excellent oral communication skills both in‑person and telephonically, to groups and individuals.

Excellent written communication skills.

Strong critical thinking, analytic, strategic,apä innovative skills.

B.A./B.S. in a healthcare‑related field coupled with 2 years of experience in primary care, ambulatory care, or another related field.

Preference will be given to a Registered Nurse.

Preference will be given to experience completing Annual Wellness Visits.

Preference will be given for a strong background in VBC programs, quality or process improvement, data analysis, healthcare recognition/accreditation, and change management.

Able to travel when required; annual expectation of 25% travel capability.
