Aviata Health Group is looking for a qualified Traveling MDS Coordinator, RN to join our team. Must be willing to travel throughout Florida. We are searching for a Traveling MDS Coordinator who is patient & family focused, a team builder, and excited about the opportunity to assist in building a culture.

Major Responsibilities – Traveling MDS Coordinator, RN
Coordinate the entire RAI process ensuring accuracy, timeliness, and completion of the MDS, CAAs and interdisciplinary Care plans for all residents in accordance with federal and state certification guidelines.
Monitor and document management of Medicare and Managed Care residents in collaboration with facility team members.
Complete accurate and timely MDS, CAAs and plans of care in accordance with RAI guidelines for all residents.
Act in compliance with company, regulatory, and professional standards and guidelines.
Adhere to facility policies and procedures and participate in facility quality improvement and safety programs.
Be able to travel to facilities to support the MDS process as needed.
Become a mentor for MDS staff.
Provide an environment for new MDS staff to excel in the position.
Provide innovative, responsible healthcare with new ideas and concepts that continually improve systems and processes.
Minimum Qualifications – Traveling MDS Coordinator, RN
Valid license as a Registered Nurse/Licensed Practical Nurse in the state(s) of practice.
At least three (3) years of nursing experience in a setting serving the same age or type of resident population.
Preferred: two (2) years of experience as an MDS Coordinator.
Experience traveling between multiple facilities with extensive overnight travel.
Qualified, compassionate, and dedicated to delivering quality care.
