LPN / RN Case Manager Brooklyn, NY
We are seeking a dedicated and detail-oriented Licensed Practical Nurse (LPN) or Registered Nurse (RN) – Case Manager to join our growing team. The Case Manager will coordinate care, ensure compliance, and support a caseload of patients receiving private duty nursing services. This role serves as a key point of contact for patients, families, and caregivers.
Key Responsibilities
Manage a caseload of patients receiving home care nursing services.
Serve as a liaison between patients, patients’ families and insurance companies.
Develop and update individualized care plans in collaboration with RNs and other healthcare professionals.
Ensure documentation is complete, timely, and compliant with state and agency regulations.
Monitor field nurses’ performance and provide clinical support or coaching as needed.
Communicate regularly with patients and families to address concerns, care needs, and updates.
Participate in interdisciplinary meetings and case conferences.
Support emergency coverage planning or last-minute needs, including occasional home visits.
Ensure all care aligns with agency standards, patient rights, and clinical guidelines.
Qualifications
Current and valid LPN license in New York State.
Minimum 2 years of experience in home care, private duty nursing, or related healthcare setting.
Strong clinical knowledge and ability to manage medically complex cases.
Excellent communication, organization, and time management skills.
Comfortable with EMR systems and digital documentation.
Ability to work independently and collaboratively in a fast-paced environment.
Valid driver’s license and reliable transportation preferred for occasional field visits.
