• Board Eligible (within 5 years of residency) or Board Certified in Urology - REQUIRED
• Active CO License (or willingness to obtain) - (note in "Additional Submission Details" field)- REQUIRED
• BLS and ACLS - REQUIRED
• Most recent NPDB Self Query Report (within 30 days active) - REQUIRED no exemption, required at presentation
• Visa Support: J1 or H1B candidates are eligible for visa sponsorship. (note in "Additional Submission Details" field) - REQUIRED
• DEA (must be obtained prior to start) - (note in "Additional Submission Details" field) - REQUIRED no exemptions
• Document care using the Epic EMR system
