Department: Physical Therapy Job Type: Contract / Long-Term Assignment Contract Length: 13 Weeks Shift: Day Shift | 7:00 AM – 3:00 PM On-Call: No Weekends/Holidays: As per facility needs Job Description Seeking a licensed Physical Therapist (PT) for a long-term contract assignment. This role will primarily support outpatient services, with occasional inpatient acute and swing-bed rehabilitation coverage. The ideal candidate is experienced, patient-focused, and comfortable working independently in a mixed outpatient and inpatient setting. Patient Population & Case Types Outpatient (Primary Setting): General orthopedics (shoulder, back, hip, knee) Falls and balance referrals Neurological diagnoses Post-surgical rehabilitation Inpatient (Secondary Coverage): Critical-access hospital swing-bed rehab (approx. 15 patients) Orthopedic injuries Post-surgical recovery General medical rehabilitation following acute stay Caseload & Scheduling Average 15 patient sessions per day Evaluations: 60 minutes (limited to 4–5 per day) Follow-ups: 30 minutes No double booking or over-scheduling Responsibilities Evaluate, plan, and implement individualized physical therapy treatment plans Provide therapeutic exercises, manual therapy, and patient education Document care accurately and timely per facility standards Collaborate with interdisciplinary healthcare teams Ensure patient safety and high-quality outcomes Requirements Active Physical Therapist (PT) license – New York BLS certification Experience in outpatient orthopedics required Acute care or swing-bed rehab experience preferred Strong clinical, documentation, and communication skills Additional Information No on-call requirement No critical urgency Supportive rehab department Stable, predictable schedule About LanceSoft Established in 2000, LanceSoft is a Certified MBE and Woman-Owned organization.
