Sentara Health is seeking to hire a qualified individual to join our team as a
Licensed Practical Nurse. Position Status: Full Time - Day Shift Position Location: Sentara Home Care Services - Suffolk, VA ***This position will be mostly in the office, however, travel with Hampton Roads is required. Additional compensation may be available for this role such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities. The Licensed Practical Nurse (LPN) in a home health care setting is responsible for providing direct, one-on-one care to patients in their homes. This role involves monitoring patient health, administering treatments, managing medications, and offering essential support to patients with various medical needs. Collaboration with a team of healthcare professionals, including Registered Nurses (RNs) and physicians, is required to deliver high-quality, compassionate care that improves patient well-being. Trade School Graduate (Required)
LPN License (Required) - Active in Virginia Serve as patient advocate in the prevention of illness or disease Provide care and treatment for patients and clinical support and teaching for patient and family members Customer service, testing, medication administration and the implementation of nursing interventions and care plans Support a holistic approach to patient care
