Title: Integrated Behavioral Health Provider Reports to: Director of Care Coordination
Who we are: C4 was founded in 1972 to help people released from psychiatric hospitals into the Uptown and Edgewater communities stabilize in the community. Since that time, C4’s service area has expanded to rekindle hope for individuals and families on Chicago’s north and west sides, remaining committed to community-level, system-focused interventions; C4 always seeks to avoid restrictive levels of care and maximize families’ agency to lead their recovery, allowing them to heal in their homes and community. C4 is the leader in building hope and breaking down barriers – from the personal to the systemic - for Chicago’s systematically disadvantaged communities through the delivery of effective, empirically supported, and trauma-informed mental health services. C4 seeks to equip 7,000 individuals annually to help them move from trauma to living, to working and thriving within their community.

Purpose :
Under the general supervision of the Director of Care Coordination and the Chief Clinical Officer, the Integrated Behavioral Health Provider is responsible for delivering evidence-based behavioral health services. As part of an integrated behavioral health and residential practice, their duties include conducting initial patient evaluations, performing triage assessments, managing crisis interventions, providing short-term individual and group counseling, offering skills training, coordinating care, and conducting follow-up sessions.They participate in interdisciplinary case conferences, work with primary care providers, and assist with program development.
Minimum
Qualifications/Credentials:
Master’s degree in social work, counseling, or marriage and family therapy Must be Licensed Practitioner of Healing Arts (LPHA) Must be Certified Drug and Alcohol Counselor (CADC) Two (2) years of progressively responsible experience in mental health services. Bilingual in Spanish/English is highly desirable.
Competencies (Minimum Skills, Knowledge and Experience):
Ability to be respectful of the diverse cultures of the people served and to provide culturally appropriate, competent and individualized treatment according to each client's age, gender, race, ethnicity, and culture. Clinical competencies, including motivational interviewing techniques, as aligned with training mandated by agency Ability to independently assess the psychosocial functioning and needs of patents and their family members of all ages. Ability to provide consultation services to staff about psychosocial needs to patients and the impact of psychosocial problems on health care and compliance with treatment Demonstrate effective time management to organize work, establish priorities, meet deadlines, and maintain flexibility in situations involving change or implementation of new procedures with positive disposition. Maintain quality control standards. Analyze and pay attention to detail Well-versed in Rule 132/140 and SUPR 2060/ 2090 Experience navigating nonprofit behavioral health service delivery and documentation
Duties and Responsibilities:
Evaluate the ongoing necessity for service delivery, progress in services and make recommendations for treatment changes as appropriate Verify clinical need for services and treatment recommendations Coordinate with treatment teams to gather additional clinical information necessary Work closely with clinical supervisors to address questions or concerns related to staff training in best practices for clinical documentation Advise clinical staff regarding appropriate procedures or practices in response to completion of assessment tools Participate in clinical staffing and provide guidance on best practices and client engagement. Development and updating of a comprehensive referral list of community services. Provide and/or oversee referrals to community resources. Develop and maintain relationships and good communication with related health, educational, and social services agencies Participates in a comprehensive quality improvement process, which includes routine auditing of medical records to ensure quality clinical care, compliance with clinic protocols, and implementation of the Board's quality assurance committee recommendations concerning integrative behavioral health care Other duties as assigned by supervisor
