***** Active RN license in the practicing state. Experience in OB/GYN nursing or case management preferred. CCM or OB/GYN care management certifications preferred.
***** Assess patient needs and develop individualized care plans for OB/GYN patients. Coordinate care with OB/GYN providers, specialists, and multidisciplinary teams. Monitor patient progress and adjust care plans as needed. Educate patients and families on prenatal, postpartum, or gynecologic care and resources. Maintain accurate documentation and ensure compliance with regulatory and quality standards.
