Position Overview Greenlife Healthcare Staffing is seeking a compassionate Family or Adult Geriatric Nurse Practitioner (NP) to join a nursing home in Brooklyn, NY. This role is open to new graduates and experienced NPs, offering flexible scheduling and the opportunity to deliver patient-centered care to elderly residents in a collaborative, mission-driven environment.

Technical Skills: Proficiency in EHR systems, chronic disease management, and infection control protocols.
Soft Skills: Strong communication, empathy, cultural competency, and attention to detail.
Key Responsibilities
Conduct comprehensive patient assessments, including medical history reviews and physical exams.
Develop and update individualized care plans for residents with acute/chronic conditions.
Monitor and manage infections by adhering to facility protocols.
Educate patients and families on disease management, prevention, and wellness.
Collaborate with physicians, nurses, and therapists to ensure coordinated care.
Maintain accurate documentation of treatments, progress, and care plans.
