About the Role Concord Post Acute is seeking a dedicated and compassionate PRN Physical Therapist Assistant (PTA) to join our rehabilitation team. In this role, you will provide vital support under the direct supervision of a licensed Physical Therapist, assisting with patient-related activities to help our residents regain mobility, manage pain, and achieve their rehabilitation goals.
Essential Duties & Responsibilities
Administer physical therapy treatments and procedures as directed by the Physical Therapist.
Document daily treatment notes, weekly progress notes, and other required records accurately Products and conduct timely, in accordance with state regulations.
Assist with the maintenance and cleaning of the therapy department and equipment.
Participate in patient care and rehabilitationplexes conferences as needed.
Train restorative aides in the nursing department as instructed.
Communicate effectively with the Physical Therapist, supervisor, and other healthcare team members regarding patient progress, concerns, and treatment plans.
Educate patients, their families, and nursing staff on at-home maintenance programs and caregiver training in preparation for discharge.
Report any issues with departmental equipment to ensure it remains in good working order.
Maintain a positive, effective, and professional rapport with patients, residents, families, and colleagues.
Qualifications
survival>Active and current Physical Therapist Assistant (PTA) license in California.
