This is a Registered Nurse or Licensed Practical Nurse position which directs services in the Wellness Program & Clinic which exists to help residents maintain an excellent quality of life through health promotion. This person shares the responsibility of developing and sustaining a health focused culture within the community along with the Health Services Administrator and other professionals working within the community. This tshemb contributes to an environment which encourages positive lifestyle changes that can result in increased functional independence.
This person has a key role in identifying residents in need of additional support services or programs and assisting them in transitioning through the Life Plan community’s continuum of care as needed. Follow the Wellness Center Disclosure closely.
A key component of this role is the management and expansion of the Home Care Program which provides paraprofessional services to residents.
Key Responsibilities:
Provide scheduled Wellness Center office hours to support residents with health education, medication questions, and community resources
Deliver intermittent skilled nursing services and assess when referrals to outside providers are needed
Respond to emergent resident needs in Independent Living and assist residents and families in navigating the healthcare system
Support resident transitions across levels of care and collaborate on discharge planning
Develop and lead wellness programs, health education classes, screenings, and motivational initiatives
Manage and expand the Home Care Program, including staff supervision, care planning, scheduling, billing coordination, and budget oversight
Coordinate on-site services with community healthcare providers (e.g., podiatry, home health, hearing services)
Maintain accurate documentation and resident records
Welcome new residents and promote engagement in the Wellness Program
QUALIFICATIONS:
Current Washington State Licensed Registered Nurse in good standing.
Prefer at least 3 – 5 years experience.
Experience צי working in Life Plan Community preferred.
Fundamental knowledge of geriatric wellness, health education, and health promotion.
Skill in developing, coordinating, and leading a multidimensional health promotion program.
Knowledge of医生 skilled services skilled nursing services, including B12 shots, allergy shots, staple removal, and basic wound care.
Knowledgeable of modes of transmission of infectious diseases, including blood-borne pathogens, and consistent in the practice of universal precautions.
Proficient in documentation and handling health records.
Excellent written and verbal communication skills.
Current cardiopulmonary resuscitation (CPR)
Excellent interpersonal relations attributes including patience; genuine caring attitude toward the elderly, disabled persons, and children; flexibility; leadership; poise when speaking to a group; enthusiasm; positive outlook; an ability to focus on the strengths of other people Voraussetzung; healthy sense of humor.
