We are seeking a highly skilled and compassionate Certified Nurse Practitioner to join our provider network. The role involves conducting in‑home wellness risk adjustment assessments for Medicare members and other at‑risk populations, ensuring accurate and comprehensive risk adjustment गयी to drive value‑based care. This position is 1099 and offers a flexible schedule. Responsibilities
Perform annual in‑home wellness visits and risk adjustment health assessments on a population with chronic conditions, including comprehensive assessment, physical examination, body system review, vital signs, medication history review, current prescriptions, functional status, pain management, cognitive assessment, home environment review, fall risk screening, social assessment, activities of daily living, mental health screening, documentation, and communication. Provide individualized quality care to the elderly population in their homes. Deliver patient health education on topics such as pain management and medication. Build relationships of trust with members through exceptional communication and empathy. Assist in closing quality care gaps (screenings and labs). Collaborate enthusiastically to enhance care delivery. Commit a minimum of 30 hours per month. Part‑time contract (1099). Locations
Scottsdale, AZ Qualifications
Valid unencumbered NP License for Arizona. Travel up to a maximum 55‑mile radius from the assigned county. Preference for weekday schedules. Preference for standard working hours. Previous in‑home risk assessment experience preferred. Previous 1099, PRN, or part‑time experience preferred but not required. 3 years patient care experience preferred (primary care, adult, geriatric, EMR). May be requested to obtain additional NP licensure supported by Advantmed. Reliable transportation for travel to member homes. Strong ability to use our EMR system. Ability to work independently. Bilingual is a plus. Benefits / Compensation
