is currently seeking an experienced
Physical Therapist (PT)
for a
full‑time or part‑time
position performing home‑health visits.
The office services adult and geriatric clients on a per‑visit basis in the Williamsburg to Yorktown, VA area.

Responsibilities
Make home visits to clients in designated geographic territories.
Perform diagnostic tests and measurements, such as range of motion, transfer status, stability, gait pattern, muscle strength and endurance, balance testing, and safety assessments.
Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence‑based dressings and topical agents.
Develop and implement individualized care plans, including manual therapeutic վտանգ, gait training, balance, and other interventions.
Continuously assess and revise the PT care plan, and collaborate with nursing in the multidisciplinary care plan as appropriate.
Educate and instruct clients, family members, or other representatives in rehabilitative care and activities that promote independence.
Accurately document observations, interventions, and evaluations using a state‑of‑the‑art touch‑pad tablet.
Qualifications
A current
VA
Physical Therapist‑PT license.
At least one year of recent work experience as a PT.
Graduation from an APTA‑approved program or an AMA body‑approved program, documented by transcript or diploma.
Ability to work independently and manage time effectively.
Strong interpersonal, organizational, and problem‑solving skills.
Solid computer skills; prior EMR experience preferred.
