Tapestry 360 Health is seeking a passionate and dedicated Physician to join our mission-driven team, committed to providing quality, culturally competent primary care to underserved communities. Our physicians play a pivotal role in enhancing community health through accessible, prevention-oriented, and collaborative healthcare services.
Essential Duties and Responsibilities: Patient Care : Conduct comprehensive health assessments, including physical examinations, ordering diagnostics, and developing individualized treatment plans. Prevention and Treatment : Emphasize disease prevention, health maintenance, and holistic patient wellness across all age groups. Collaborative Practice : Work in a team-based care model with nurse practitioners, physician assistants, and allied health professionals, ensuring cohesive and integrated care. Community Focused : Engage in outreach, patient education, and preventive care initiatives to reduce health disparities within the community. Documentation : Accurately document all patient interactions, diagnoses, treatments, and follow-up care in an electronic health record (EHR) system. Quality Improvement : Participate in quality improvement initiatives, peer review, and evidence-based practice updates to continuously elevate patient care standards.

Physicians at Federally Qualified Health Centers (FQHCs) require a unique set of competencies to effectively serve diverse and underserved populations.
Clinical Expertise in Primary Care Proficiency in diagnosing and managing a broad spectrum of medical conditions across all age groups. Ability to perform comprehensive physical examinations, interpret diagnostic tests, and develop individualized treatment plans.
Cultural Competence and Sensitivity Understanding and respecting diverse cultural backgrounds, beliefs, and practices. Ability to communicate effectively with patients from various cultural and socioeconomic backgrounds, ensuring language barriers are addressed.
Interdisciplinary Collaboration Experience working within multidisciplinary teams, including nurses, social workers, behavioral health specialists, and community health workers. Ability to coordinate care and collaborate on treatment plans to provide holistic patient care.
Commitment to Health Equity and Advocacy Dedication to addressing healthcare disparities and improving health outcomes for underserved populations. Advocating for patients' needs within the healthcare system and supporting initiatives that reduce barriers to care.
Adaptability and Resilience Flexibility to adapt to varying patient volumes, resource limitations, and evolving healthcare environments. Resilience in managing complex cases and navigating the challenges inherent in community health settings.
Qualifications:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below represent the knowledge, skill, and/or ability required. Required Qualifications : Education : Doctor of Medicine (MD) or Doctor of Osteopathy (DO) from an accredited medical school. Licensure & Certification :
Active Illinois medical license. Board certification or eligibility (to be completed within two years). Documentation of completed residency.
Experience : Proven ability to manage both common and complex health conditions within a community health setting. Experience with underserved populations is highly desirable. Skills :
Strong analytical, problem-solving, and interpersonal skills. Proficiency with electronic medical records (EMR) systems. Ability to work effectively within a multidisciplinary team.
Salary and Benefits: Compensation: The annual salary range for this position is $160,000.00 and $200,000.00, based on experience and qualifications. Benefits: Tapestry 360 Health offers a comprehensive benefits package including health insurance, dental insurance, retirement savings plans, paid time off, and continuing education support. Tapestry 360 Health will cover the following expenses: hospital dues, Illinois Physician and Controlled Substance License and Registration Fees, as well as Examination Fees for Board Certification and Recertification.

In accordance with the Illinois Pay Transparency Act, we are disclosing the full salary range for this position. This range represents the potential compensation for the role based on experience, tenure, and performance over time. Most new employees can expect an initial offer within the lower portion of the range, reflecting factors such as prior experience, internal equity, and organizational budget.
