SmartCare360.Health is seeking a Patient Support Coordinator (PSC) + Health Coach to serve as the bridge between patient enrollment and care delivery. This role is responsible for ensuring patients successfully transition from enrollment into active care, reducing drop-offs, and improving overall patient engagement and outcomes. You will support patients throughout their care journey, while also providing health coaching and assisting with clinical workflows. This is a hybrid role combining patient coordination, coaching, and medical assistant responsibilities.
