Provide comprehensive assessments, diagnoses, and ongoing management of residents in long-term care/skilled nursing facilities
Collaborate with facility staff, physicians, and interdisciplinary teams
Develop individualized care plans to support optimal patient outcomes
Maintain accurate and timely documentation
Deliver care with compassion, patience, and professionalism
What We re Looking For
Active APRN-NP license
(Required)
Experience in geriatrics, long-term care, or primary care
preferred , but
new graduates are encouraged to apply
Strong commitment to delivering high-quality, patient-focused care
Excellent communication, critical thinking, and clinical judgment
