Days (Monday-Friday) Summary: We are looking for a skilled
Nurse Practitioner (NP) or Physician Assistant (PA-C)
to provide advanced wound care services to patients across the Raleigh area. This is an excellent opportunity for clinicians who value autonomy and patient-centered care in a home-based setting. Responsibilities: Conduct detailed wound assessments and follow treatment plans Track and document healing progress using best-practice protocols Provide education to patients and caregivers on wound care techniques Coordinate with physicians and interdisciplinary care teams Ensure accurate and timely EHR documentation Travel to patient homes within Raleigh and surrounding areas Qualifications: Active NP or PA-C license in North Carolina Wound care experience is a plus New graduates are welcome to apply with relevant clinical exposure or hands-on experience in wound care settings Ability to work independently in a field-based role Valid driver's license Comfortable working in a home care environment Benefits: Paid Time Off: 2 weeks annually Paid holidays Mileage reimbursement: $0.50 per mile Stable weekday schedule Nurse Practitioner / Physician Assistant (Home Health)
