As a vital member of our home healthcare team, you will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being.This role offers mileage, full health benefits, and PTO! Employees must maintain 30 points weekly.Under the direction of the RN, assists in identifying the patient's physical, psychosocial, and environmental needs as evidenced by documentation.Participates in planning and implementing care in partnership with the patient, representative (if any), caregiver(s), and the RN.Provides services ordered by the physician as indicated in the care plan.Provides patient and caregiver education.Provides care using infection control measures that protect both staff and patients.Assures the continuity of care through the delivery of quality patient care.Provides effective communication to the patient, family, team members, and other healthcare professionals as evidenced by clinical notes, case conferences, and communication notes.Completes appropriate documentation in a timely manner.Monitors assigned cases to ensure compliance with the requirements of the third-party payor.Participates in the Agency's QAPI program.Participates in the agency-sponsored in-service training.Demonstrates commitment, professional growth, and competency.Promotes the Agency philosophy and administrative policies to ensure quality of care.Conduct patient assessments and develop personalized care plans.Administer medications and treatments as prescribed by physiciansPerform wound care and dressing changes.Monitor and record vital signs, symptoms, and patient progress.Educate patients and their families on disease management and preventive care.Collaborate with other healthcare professionals to ensure comprehensive patient care.Maintain accurate and up-to-date electronic health records.Adhere to all home health care regulations and company policies.Participate in case conferences and quality improvement initiatives.Manage time effectively to provide care for multiple patients throughout the day.QualificationsCurrent Licensed Vocational Nurse (LVN) certification in the state of TexasValid Basic Life Support (BLS) certificationStrong clinical skills in patient assessment, medication administration, and wound careProficiency in using electronic health records (EHR) systemsExcellent time management and organizational skillsOutstanding communication and interpersonal skillsAbility to work independently and as part of a multidisciplinary teamKnowledge of home health care regulations and best practicesUnderstanding of common chronic conditions and their managementPrevious experience in home health care preferredExperience working with diverse patient populationsReliable transportation and valid driver's licensePhysical ability to perform essential job functions, including lifting and transferring patientsAdditional InformationAll your information will be kept confidential according to EEO guidelines.
