Ability to work cooperatively as a team leader. Supervisory experience preferred. Responsibilities:
Collaborate with the clinical team to evaluate prospective participants and handle documentation for initial MassHealth authorization. Oversee admission processes, including assessments, documentation, medication plans, and care planning during attendance. Develop and review multi-disciplinary care plans, conduct yearly assessments, and update care plans accordingly. Generate and supervise MD orders for participants upon admission and every 6 months. Maintain medication records, administer medications, and monitor glucose in compliance with regulations. Document nursing notes, incident reports, and emergent issues. Provide direct and supervised medical care, including assessments, counseling, medication administration, vital signs, and care coordination. Supervise Activities of Daily Living assistance, including toileting, ambulation, and transfers. Conduct health groups and oversee meal compliance with dietary and medical needs. Participate in serving lunch and oversee related documentation for MassHealth and SCO authorizations. Train and supervise staff on OSHA and health standards, and coordinate staff training on health issues and care techniques. Maintain nursing competencies and supervise nursing and medical students. Review and approve nursing timesheets bi-weekly. Attend relevant meetings and monitor medical supplies. Collaborate with leadership on policy and client care issues, and work outside scheduled hours as needed. Qualifications:
