We are able to accomplish this by partnering with schools, outreach programs, and other college and career access organizations to provide efficient and effective college and career access programming and classroom-based test preparation. POSITION: College Transition Case Manager Study Smart Tutors, Inc. is seeking a detail-oriented and personable College Transition Case Manager to support recent GEAR UP graduates in their postsecondary journeys. This role is ideal for someone who is passionate about student success, highly organized, and skilled at building trust with first-generation college students. JOB RESPONSIBILITIES: The College Transition Case Manager provides individualized support to high school graduates transitioning to college by serving as their primary point of contact. They build strong relationships through regular outreach, assists with enrollment steps, and monitors academic progress. The Case Manager maintains accurate documentation, tracks engagement, and submits monthly reports. This role requires balancing proactive communication, effective case management, and timely referrals to appropriate campus resources. Serve as the primary contact for a caseload of students transitioning from high school to college Build relationships through consistent outreach via phone, text, email, and virtual/in-person meetings Support students with enrollment steps such as FAFSA completion, class registration, and orientation Track all student interactions using outreach logs and maintain accurate case notes Monitor academic progress, flag early warning signs, and make timely referrals to campus resources Submit timely and thorough monthly reports highlighting student needs, engagement, and outcomes Use time efficiently to balance proactive outreach, documentation, and responsive support QUALIFICATIONS: Bachelor’s degree is required Preferred to be located in San Diego or able to travel regularly within San Diego County, including to local colleges Excellent attention to detail and ability to document data accurately Conduct in-person check-ins or campus visits with local students as needed Visit local college campuses to build connections and coordinate student support Strong interpersonal and communication skills; personable and student-centered Demonstrated ability to manage time and caseload efficiently Experience working with first-generation students or in student support roles PREFERRED QUALIFICATIONS: Background in counseling, education, social work, or student affairs Familiarity with support services in the California Community College, GEAR UP, and/ or CSU/UC systems SALARY AND BENEFITS: No cost virtual counseling and therapy services Employee Referral Program bonus eligibility PTO Study Smart Tutors is an equal opportunity employer and does not discriminate in its hiring of employees on the basis of race, religion, gender or sexual orientation.
