Work with the direct support staff/job retention counselors and individuals to implement their Individual
Plan within the community employment program integrating them into employment, job skills development
and/or job exploration. Assist with job coaching activities for people with developmental disabilities
including person centered planning, job analysis, job placement, job customizing, training and supports for
successful job match and job retention. Act as a facilitator at team meetings as needed; Post Individual
Plan development activities to be completed within 20 days of the meeting: Assist with training direct
support staff/ job retention counselors on how to document data in the Efforts to Outcome System; Ensure
and/or provides applicable training for direct support staff/ job retention counselors to aid in facilitating
individual's goals.Maximize individuals' participation in all decision-making processes. %
2. Develop the program/off-site route schedule, activities and training programs that promote community, social
and employment skills that lead to employment experience and/or job placements. Provide reports of
program activities, mileage submissions, invoices, schedules, etc. to the Program Director on a weekly
basis.Train direct support staff/ job retention counselor to identify and communicate strengths and areas of
improvement for individuals and possible methods of enhancing strengths and minimizing weaknesses.
Provide on-going oversite of Data Collection in the Efforts to Outcome System. %
3. Provide immediate support in crisis intervention in areas such as, but not limited to immediate on-site
intervention, problem analysis and initial recommendations.Respond to inquiries/requests from persons
served, family, caregivers, peers and all other internal and external customers in a timely manner. %
4. Facilitate individual's self-esteem through positive interactions and building a relationship of trust. Ensure
individuals' fundamental rights (COMAR 10.22.04) and satisfactory conclusion to incident reports in a
timely manner. %
5. Adhere to State of Maryland DDA, CARF and Melwood policies and procedures. %
6. Actively participate in interdisciplinary team and staff meetings. Maximize individuals' participation in all
decision-making processes. %
7. Adhere to and further the mission of Melwood by creating a supportive environment for participants and staff. %
