Position Summary
The
MDS Coordinator
is responsible for coordinating and overseeing the completion of the Minimum Data Set (MDS) assessments and care plans for residents in accordance with federal and state regulations and facility policies. This position ensures accuracy and timeliness in data collection to support resident care, reimbursement, and regulatory compliance.
Essential Duties and Responsibilities Complete, coordinate, and monitor timely and accurate MDS assessments (comprehensive and quarterly). Develop and revise individualized care plans in collaboration with the interdisciplinary team. Ensure compliance with CMS regulations and RAI guidelines. Monitor changes in residents' conditions and coordinate significant change MDS assessments. Participate in care plan meetings, resident assessments, and quality improvement initiatives. Submit MDS data to appropriate federal/state agencies per required schedules. Review and interpret clinical records and documentation to support accurate coding and reimbursement. Train and support nursing staff in documentation best practices related to MDS. Work closely with billing and reimbursement teams to ensure accurate RUG/PDPM classification.
Knowledge, Skills and Abilities Strong knowledge of RAI process, MDS 3.0, PDPM, and federal/state regulations for long-term care. Excellent assessment, documentation, analytical, and communication skills. Proficient in EHR systems and MDS software (e.g., PointClickCare, MatrixCare) RAC-CT certification (Resident Assessment Coordinator - Certified) - Preferred Strong organizational and leadership skills
Education & Experience Current RN or LPN license in CA. Minimum 1-2 years of experience in MDS coordination within a long-term care/SNF setting Experience with PDPM and quality measure reporting - Preferred
