The LVN performs focused assessments, participates in care planning and health teaching, administers medications, and ensures safe and ethical practice. The role supports patients across various age groups, primarily focusing on adults and geriatric populations, ensuring quality outcomes and adherence to regulatory and ethical standards.Key Responsibilities:Collect data and perform focused nursing assessments of individual health status.Participate in planning, developing, and modifying nursing care plans.Assist in evaluating patient responses to nursing interventions.Provide health teaching and counseling for optimal wellness.Administer medications/treatments safely, knowing rationale and effects.Accurately document nursing care, interventions, and patient responses.Report adverse effects and communicate clearly with health professionals.Use evidence-based information to contribute to interdisciplinary policies.Clarify questionable medication/treatment orders with appropriate providers.Promote and implement safety measures for patients and staff.Seek instruction/supervision for unfamiliar procedures or equipment.Notify supervisor upon departure from assignment.Maintain professional boundaries and comply with ethical mandates.Fulfill mandatory reporting obligations under the Texas Nursing Practice Act.Deliver care without discrimination based on personal characteristics.Prevent exposure to infectious conditions and maintain infection control.Take responsibility for professional development and competence.Accept only appropriate nursing assignments that ensure safety.Support and comply with state, federal, and facility regulations.Maintain confidentiality and protect patient privacy.Participate in client education and family counseling.Perform additional duties as assigned to support organizational goals.Supervisory Responsibility:This position does not supervise staff but may serve as a mentor/preceptor to nursing students or newly hired LVNs.Minimum Job Requirements:Licensure: Active Texas LVN license without restrictions.Certification: Current Basic Life Support (BLS/CPR) certification.Education: High school diploma or GED; graduate of an approved LVN program.Language: Must be able to read, write, and communicate effectively in English.Preferred (Geri-Psych): Experience in Emergency Room or Psychiatric settings.Knowledge, Skills, and Abilities:Strong communication skills to interact with patients, families, and care teams.Sound decision-making and critical thinking skills, especially under pressure.Effective time management to handle multiple patient needs.Proficient computer skills for electronic medical record (EMR) use.Demonstrated clinical competency in LVN scope of practice.Ability to supervise support staff and contribute to a team environment.Other Essential Functions:Complete required education, competencies, and training by deadlines.Maintain regular and punctual attendance; participate in meetings.Promote safety, follow safety protocols, and report incidents promptly.Uphold the organization's Mission, Vision, and Code of Conduct.Adhere to departmental and facility policies, OSHA, TJC, CMS, HIPAA regulations.Provide exceptional customer service and foster a professional team culture.
