Your Role and Responsibilities: Coordinate and direct care for a diverse caseload of home health patients requiring physical therapy. Conduct comprehensive assessments to develop individualized patient care plans in collaboration with the patient, family, and physician. Educate patients, families, caregivers, and community providers on care best practices. Monitor and evaluate patient progress to ensure goals are achieved effectively. Engage in interdisciplinary conferencing to optimize patient care. Identify performance improvement opportunities and assist with implementing necessary changes. Qualifications: Minimum of one year of clinical physical therapy experience. Graduated from an accredited physical therapy program. Possess an active Physical Therapist license and American Heart Association BLS certification. Valid Driver's License.
