Below are more details before you decide to apply:Location: onsite in Jacksonville, FLPosition: Healthcare NurseSchedule: 12 hour Shifts: Week 1: Sun-Tues, Week 2: Sun-WedHourly: $26.50Benefits start after 30 days: Health, Dental, Vision and many more!All offers of employment are contingent upon background screensPosition Summary:The Licensed Practical Nurse (LPN) plays a key role in the interdisciplinary care team at our residential neurorehabilitation program. This position is responsible for providing direct nursing care, supporting medical coordination, and promoting wellness and safety for residents with acquired brain injuries (ABI), traumatic brain injuries (TBI), and related neurological conditions.The LPN supports continuity of care through consistent monitoring, documentation, communication, and health education within the scope of LPN practice and under the guidance of the Medical Director. The LPN reports directly to the Director of Program Operations and works closely with the Residential Supervisor and Medical Assistant to ensure timely medical follow-up and smooth coordination of care.This role includes working with a diverse population, including military veterans, and individuals with co-occurring psychiatric conditions commonly associated with TBI, such as PTSD, depression, anxiety, and mood disorders. Experience in managing addiction and substance use-related concerns is highly preferred. A trauma-informed, collaborative approach is essential to supporting the complex medical and behavioral health needs of this population.Essential Duties and Responsibilities:•Conduct monthly nursing assessments for all residents, including documentation of current medical status, any changes in condition, and other relevant clinical observations. Submit completed assessments to the medical record.•Monitor resident health status and triage medical concerns in collaboration with the interdisciplinary clinical team.•Coordinate and attend resident medical appointments to ensure continuity of care. Provide appropriate follow-up education and communication to staff.•Work directly with the Residential Supervisor and Medical Assistant to coordinate all annual doctor appointments and ensure proper documentation and follow-up.•Maintain regular and professional communication with family members, case managers, program coordinators, the clinical team, and the Medical Director.•Schedule and coordinate necessary follow-up care, diagnostic tests, and specialty consultations as indicated by resident status.•Provide individualized health education to residents and staff, based on specific client needs and medical conditions.•Lead monthly in-service trainings and staff meetings that include health education topics relevant to neurorehabilitation and residential safety.•Assist in the onboarding of new Certified Nursing Assistants (CNAs), ensuring they receive appropriate training in medication protocols, resident safety, and general medical procedures.•Provide training and ongoing competency evaluation to CNAs on safe and correct medication administration procedures, including the use of the medication system (or equivalent). Ensure all documentation is submitted to HR.•Provide medication education to residents, including understanding their prescriptions, side effects, and the importance of adherence.•Supervise the administration of medications according to physician orders, monitor for effectiveness and side effects, and document resident responses.•Complete annual Health Appraisals for all residents in accordance with CARF accreditation and state licensing regulations.•Monitor medication carts and supply levels to ensure medications are accurately ordered and available. Maintain strong communication with the pharmacy to prevent gaps in medication access.•Attend weekly staffing meetings, clinical reviews, and team coordination meetings as an active treatment team member.•Assist physicians by providing resident updates, coordinating physical exams, and facilitating communication between the medical team and residential staff.•Complete higher-level medical tasks and skilled nursing duties for residents that are outside the scope of practice for CNAs, such as wound care, injections, medication adjustments per physician orders, and clinical assessments.Qualifications and Education Requirements:•Current and valid LPN license in the state of Florida (in good standing)•Minimum of 3 years clinical nursing experience (experience in neurorehabilitation, brain injury, or residential care preferred)•Working knowledge of commonly prescribed medications and treatment approaches in neurobehavioral or behavioral health populations•High ethical standards and professional nursing judgment•Excellent interpersonal, written, and verbal communication skills•Ability to multitask, manage priorities, and document clearly in an electronic medical record (EMR) system•CPR and First Aid certificationPreferred Qualifications:•Experience working with individuals with TBI, ABI, developmental disabilities, or complex medical and behavioral conditions•Experience working with individuals with co-occurring psychiatric conditions (e.g., PTSD, depression, anxiety) and substance use disorders, especially within neurorehabilitation or veteran populations.•Familiarity with CARF standards and residential care regulatory requirements•Proficiency in using the medication management system (or similar electronic med tracking systems)•Knowledge of trauma-informed care and person-centered planning principles Equal Opportunity Employer/Protected Veterans/Individuals with Disabilities This employer is required to notify all applicants of their rights pursuant to federal employment laws.
