Requirements:
Board Certification
Certifications: ACLS, BLS, DEA
Licensed in Virginia
Fellowship Trained - HIGHLY PREFERRED
Surgical Procedures Required: Thyroidectomy, Extended Parotidectomy
General Surgical Otolaryngology Requirements: Tonsillectomy, Adenoidectomy, PE Tube
General Non-Surgical Otolaryngology Requirements: Hearing Evaluation, Eval and Manage Disorders Involving Ears, Nose, Throat and Skeletal Facial Structures
Endoscopic Procedures Required: Septal, Sinus, Bronchoscopy - Flexible, Bronchoscopy - Rigid, Laryngoscopy
Facial Surgery Procedures Required: Rhinoplasty, Facial Fractures, Skin Grafts/Flaps

Assignment Details:
EMR System: Epic
Annual ER Volume: 3
Patients Per Day: 6
Credentialing Timeframe: 120 days
