Job Description Job Title: Certified Occupational Therapy Assistant Profession: Rehabilitation Therapy Specialty: COTA Duration: 13 Weeks Shift: Monday-Friday, Days Hours per Shift: 32 Experience: New Graduates welcome; SNF experience preferred License: State license verification required Certifications: CPR certification and negative drug screen results required
Must-Have: - Completed prescreen form - Resume and license verification attached - References (3) - Criminal background checks (county, federal, and multi-state for 7 years) - Nationwide sex offender search - HHS/OIG search - GSA search - Social Security number trace - PPD or Quantiferon test with proof of positive PPD and chest X-ray
Description: Ensure that the prescreen form is filled out and attached upon submission. All required documentation, including a resume and license verification, must also be attached. If any items are incomplete or not attached, the candidate will be declined. Candidates with new graduate status are acceptable. Experience in skilled nursing facilities is preferred. A COVID-19 test is required 72 hours prior to the start date, with results issued 48 hours prior to starting.
