Description
The Program Manager plays a vital leadership role in fostering a kind, fun, and collaborative environment within the group home for adults with complex behavioral and medical needs. Using a person-centered approach ensures that individuals receive compassionate, high-quality care in alignment with their Individual Support Plans (ISPs) The PM will serve as the Qualified Intellectual Disabilities Professional (QIDP) and Social Worker. The PM is central to the development, integration, coordination, implementation, and monitoring of active treatment and Individual Support Plans (ISPs) for individuals on their caseload. This role involves direct supervision, workflow development, and compliance monitoring.
Lead, Manage, and Accountability Participates in the hiring and training of new staff on all three shifts, coaching, improvement plans, and performance evaluations. Implement policies and procedures that prioritize the safety, well-being, and dignity of both staff and clients. Assess training needs and engage staff in meaningful learning experiences to ensure compliance with licensing requirements. Provide collaborative supervision, ensuring daily routines align with regulatory standards while maintaining a positive and engaging work culture. Manage payroll, timekeeping, and overtime to ensure fairness and efficiency. Lead by example, inspiring teamwork, professionalism, and a shared commitment to excellence. • Ability to demonstrate excellent problem identification and resolution skills, especially in high-pressure situations.
Program Development and Oversight
Communicate with outside vendors and contractors, including security company. Arrange for assessments and provision of services recommended or required by licensing (Speech, OT, PT, Psych, Dietician) Responsible for documenting residents' quarterly progress Foster a fun, engaging, and enriching home environment where individuals can thrive Proactively address challenges, including facility safety and equipment maintenance, with a compassionate and solution-oriented mindset. Assist in coordination of all emergency evacuation procedures and ensures compliance with monthly required drills Identifies necessary changes in the program planning, monitoring systems, and staff training curriculum within the home and advocates for these changes. Stay informed through regular training to ensure compliance and promote ongoing professional growth. Develops and maintains service delivery protocols that are in compliance with current federal and state licensing and human rights regulations. Maintain quality assurance through thorough auditing and compliance checks.
Social Work
Coordinates admissions and discharges and completes all necessary paperwork. Including pre-admission meetings. Supports the family/guardians throughout their family member's stay by encouraging and assisting involvement with the individual, clarifying policies and procedures, maintaining accurate visitation list, responding to concerns/complaints raised and assisting with resolving conflicts, or consoling parents of an individual who has passed away. Plans for and processes discharges, working with the parents/legal guardians and community resources; identifies appropriate placement options, completes a smooth transition from the group home. Arranges leave of absences for the residents. Serves as back up for incident reporting and response in the CHRIS reporting system Qualified Intellectual Disabilities Professional (QIDP)
Ensures individuals receive person-centered, high-quality services that promote independence and well-being. This role involves developing and monitoring Individual Support Plans (ISPs), coordinating services across teams, and maintaining compliance with Medicaid and agency standards.
Develop and oversee individualized support plans using a person-centered approach. Coordinate services with families, staff, and external providers. Prepare restrictive plans and data to send the SCC quarterly. Obtain consent for restrictive plans and present to SCC Participate in assessments, admissions, and team meetings to support individual needs. Monitor progress, update plans, and document services using electronic systems. Support and train staff to ensure quality implementation of plans. Communicate regularly with families and program partners. Respond to concerns, address behavioral needs, and advocate for individuals. Ensure services meet regulatory standards and promote positive outcomes. Perform other duties as assigned while continuing to embrace the values of kindness, compassion, and person-centered care. Requirements
EDUCATION/EXPEREINCE
Have one of the following: o A Doctor of Medicine or Osteopathy licensed in Virginia
o A registered nurse licensed in Virginia, or
o A bachelor's degree in a human services field including but not limited to sociology, social work, special education, rehabilitation counseling or psychology, or a bachelor's degree in another field in addition to an advanced degree in a human services field.
o Masters degree in Human Services field preferred.
• A minimum of 2 years' supervisory experience in a residential setting.
• At least 3 years of experience working directly with individuals having severe to profound intellectual, developmental, and physical disabilities.
• Experience in developing and operating Intermediate Care facilities with Intellectual Disabilities (ICF/ID)
REQUIREMENTS
Must be able to sit, stand, walk, bend, and squat for variable periods of time, adhere to the Home's dress code policy, and always wear a company-issued ID badge. Ability to lift 35 pounds independently and with or without assistive equipment Must be able to be certified in First Aid/CPR and Therapeutic Options Must be able to obtain Food Handler's Card Must complete 2-day Person-Centered Thinking training through The Learning Community for Person-Centered Practices
