Established in 1976, the Supported Independent Living (SIL) program provides transitional housing, permanent housing, and community-based case management services to adults in recovery from co-occurring disorders, who demonstrate more preparedness for independent living in the community. The SIL satellite houses seek to establish small communities where residents reside with others in recovery and continue to practice independent living skills. The SIL program offers counseling, psycho-education, psychotherapy, prescribing services, resource coordination, and advocacy services to adults, who reside within Alameda County. The SIL program features an interdisciplinary team (e.g., case managers, clinical case managers, vocational specialists, prescribing providers, housing coordinators, administrative support) who aid Alameda County Medi-Cal recipients with Serious Mental Illness. Position Summary: The Licensed Vocational Nurse (LVN) is responsible for monitoring client treatment and delivery of clinical services. Qualifications: Required: Licensed vocational nurse, with a preferred minimum of two years of mental health experience. Active BLS for Healthcare Providers certification Must effectively and efficiently time manage by being highly organized, able to prioritize, plan, delegate and communicate consistently. Must be able to maintain a flexible work schedule in accordance with program and management needs. Strong written and oral skills to meet Agency expectations of high quality in the areas of service delivery, program structure and employee relations. Must be able to meet the Immigration Reform Act of 1986 requirements. Must have valid California Driver's License and a driving record acceptable by the Agency's insurance company and is required to utilize one's personal or Agency automobile to transport clients. Solid writing skills to meet the required standards of documentation. The ability to prioritize effectively on competing demands on time. Essential Functions/Skills: Oversee a panel of at least 30 mental health consumers who are on injectable medications (full panel size of all clients is 220). Fulfill orders from a prescribing provider, including delivering injections (Long-Acting Injectable / LAIs) to persons with Serious Mental Illness. Ability to travel to a client's residence to administer an injectable medication. Assist with Clozapine management, including vital signs, lab monitoring and requisitions, and reporting. Monitor, track, and schedule follow-up injections. Assist with vital signs, reconciling medications, documenting visits, and reporting abnormal vital signs or side effects to prescribing provider. Assist with Triage / drop-in and scheduled clinics by working with prescribing providers to obtain chief compliant, HPI, and follow up care. Partner with prescribing providers to provide Telemedicine services, including setting up room, configuring equipment, rooming the client, and injection administration. Ensure supplies are stocked, stored, and are identified for re-order, as needed. Problem-solve prescription medication challenges -assisting with prior authorization / TARs when needed, and refill requests. Provide front and/or back office administrative support, as needed (e.g., obtaining medical records, answering phones, copying, fax, scanning and uploading of documents, insurance benefit information). Assist with obtaining informed consent, ROIs, updating demographic, pharmacy, and beneficiary information. Triage client phone calls; escalating as appropriate to prescribing provider. Other duties as assigned.
