Job Description: Physical Therapist Assistant (PTA)
Requirements: Completion of a 2 year Physical Therapy Assistant Program
State Licensure
Supervision: Physical Therapist Assistants will report directly to the Program Manager who is responsible for
overseeing direct clinical skills, documentation skills, and compliance to all professional standards. A review of
performance will be conducted on a yearly basis. Physical Therapy Assistant will be indirectly supervised by and RPT
will carry out treatment plans as established by the RPT, and will have all documentation signed by the RPT.
Job Description: The PTA is responsible for the treatment of residents who demonstrate disorders of
musculoskeletal deficiencies, neurological conditions, balance disorders, and transfer/gait deficits. Treatment will
only be conducted after a written order is received from the physician. • The PTA is responsible for the screening of residents of the facility as follows: • New Facility resident: Within 24 hrs. of admission • Long Term residents: Upon receipt of a "significant change" form • And/ or upon referral from nursing. • The PTA is responsible for the completion of all necessary documentation, which includes but is not limited to
the following items • (All documentation will be co-signed by RPT) • Initial Evaluation Forms • Weekly progress notes • Monthly Recertification form • Discharge Summary • All other required forms as designated by the facility and Manager • The PTA is responsible for attending all required facility meetings and in services, including if need be Medicare
meetings be, Patient staffing, family meetings, and care plans. • The PTA will in cooperation with the program manager develop programs appropriate to the needs of the
residents in the facility, to assist them achieving their highest feasible level of functioning. • The PTA will assist the facility in meeting the standards necessary to obtain state, federal and other appropriate
certification • The PTA will employ and follow all appropriate facility infection control policies and procedures and will respect
and follow OBRA Residents rights guidelines. • The PTA is responsible for the scheduling and completing of in-services for facility and staff education. • The PTA will represent her/himself in a professional manner at all times including professional conduct,
professional communication and appropriate dress while in the facility. • The PTA is responsible for other duties assigned by the PT Manager/Director and facility
