Dementia Program Coordinator

to join our skilled nursing facility in

Warsaw, IN.

The

Dementia Program Coordinator

will ensure resident needs are appropriately met, act as a liaison, and continually develop/implement and improve the programming. The ultimate goal is to provide a safe, secure environment for each resident that will promote diversity of the resident's needs, validation, socialization, psycho-social well-being, and enhance/maintain the individual's diverse needs and abilities.

Responsibilities: Educate and provide formal training to staff on dementia and related disease processes Provide ongoing training by sharing information with staff on new trends in dementia care Arrange for comprehensive orientation for new employees to ensure Orient residents and families to the facility environment; assists both in making necessary adjustments Provide individual and/or group help for residents and families during periods of adjustment, crisis, or particular need. Address family complaints and related issues in a timely and professional manner, involving facility Administrator as needed. Maintain all relative documentation Assess all residents prior to admission to determine the appropriateness of placement. Meet with Administrator, Director of Admissions, and Director of Nursing to exchange information with an interdisciplinary team on possible admission. Develop social histories and social assessments of residents Plans and coordinates a therapeutic program schedule, which meets the resident's spiritual, social, emotional, physical, and intellectual needs. Qualifications:

Must have a degree from an accredited educational institution in a health care, mental health, or social service profession, or be a licensed health facility administrator. Must possess or have the ability to obtain a social services designee certification.

Social Work background or Activity Director's Certification preferred. Must have a minimum of twelve hours of dementia-specific training, which is required within the first three months of holding the position of Dementia Program Director. Annually, a minimum of six hours of dementia-specific training will be required. Specialized study or advanced degree preferred. Must have a minimum of one year of work experience with dementia or Alzheimer's patients within the past five years.
