We are seeking a dedicated Nurse Practitioner to join our nursing home team in Waterloo, NY. This full-time, direct hire position is tailored for a compassionate practitioner experienced in adult or family care, particularly comfortable working in a long-term care setting. Salary:
$115,000-$135,000 per year
Essential Duties and Responsibilities: Provide comprehensive patient care and establish a long-term care plan for adults in a nursing home setting. Collaborate with healthcare professionals to monitor the health conditions of patients and adjust treatments as necessary. Conduct routine check-ups on patients to assess their health condition and discover possible issues. Give appropriate advice for healthy habits (diet, hygiene etc.) and preventative actions to promote overall health. Coordinating with onsite child day care services as part of family care initiatives. Participate in the analysis, development, and implementation of strategic healthcare service improvements. Oversee medical interventions administered by team members and other practitioners. Minimum Position Qualifications:
Must have an active NP Certification. Possession of a NYS Nurse Practitioner License. Graduate from an accredited Nurse Practitioner or Physician Assistant program. Proven experience in geriatric, adult, or family practice, preferably in long-term care environments. Strong interpersonal and communication skills. Ability to work autonomously and in a team environment. Preferred Qualifications:
Experience with patient care in long-term residential settings. Familiarity or certification in palliative care practices. Demonstrable leadership or management experience in a healthcare setting. Experience in handling chronic illnesses common among elderly patients. Active participation in community-based health initiatives. Advanced training in diagnostics and patient care management systems. Skills:
Excellent patient and family communication skills to foster trusting relationships. Detailed knowledge and hands-on experience with geriatric health needs and interventions. Strong organizational skills and ability to manage multiple priorities. Empathy and resilience, with an ability to manage emotional situations. Proficient in using healthcare software and systems.
