Job Description

PURPOSE OF POSITION

The Social Worker on the Psychiatry PES is a member of the Multi-Disciplinary Treatment team. The Social Worker in coordination with the Multi-Disciplinary Treatment Team ensures compliance with the department's policies and procedures conducts assessments and re-assessments; participates in the treatment plan and ensures the coordination of timely, services moving forward.

DUTIES& RESPONSIBILITIES • Conducts interviews with patients and/or family members(s)/significant others(s) to determine continuing care needs. • Completes Social Work Comprehensive Assessment / Admission Notes for all patients referred or in need of social services in accordance with departmental high/moderate risk criteria policy. • Complies with social work-psychiatry departments' documentation and it requirements. • Provides psychosocial information for patients transferring to other units to ensure continuity of care. • Provides supportive counseling, crisis intervention, and grief counseling to patients and their families/significant others as appropriate. • Assists patients with adjustment to treatment, including problems of depression, anxiety, denial, and/or anger; noncompliance with medical regimen. • Devices/implements a social work treatment/continuing care plan involving the patient, family/significant other, and the interdisciplinary team and documents the patient's medical record in accordance with the departmental policy. • Provides information, referral, and advocacy to community agencies. • Advocates and serves as a liaison for patients with community agencies. • Coordinates individual interdisciplinary case conferences as needed. • Provide task supervision to caseworkers when designated. • Provides direct/task supervision to social work students when designated. • Identifies, evaluates, and designs proper safety plans for victims of crimes/abuse, i.e.: child abuse, elder abuse, domestic violence, rape, and sexual assault. • Maintains and completes statistics of case activities as outlined in the departmental policy. • Monitors his/her performance by reviewing medical records for appropriate documentation. • Attends individual, unit, departmental, and intra/inter-hospital meetings as mandated by the supervisor and/or department director. • Assists/participates in presentations, training, and in-services as required. • Participates in the department's cross-training program. • Coordinates and executes discharge plans for hospital patients. • Responds and participates in external and internal disasters when indicated/assigned by supervisor. • Demonstrates Knowledge in gathering collateral Psychiatric History information. (Cont'd) Performs Suicide Risk Assessment and Management on the unit. • Reviews Electronic Medical Record. • Performs Shift hand-off. • Demonstrates Knowledge of and ability to utilize Psyckes database. • Performs other duties assigned by supervisor as needed.

Minimum Qualifications

For Assignment to Level III 1. Valid license as an LCSW issued by the NYSED.
