As a Care Manager / Scheduler, you will ensure the seamless coordination of home health services and engage in care management. You'll manage schedules, interact with healthcare professionals, and support patients in improving their quality of life.

Qualifications:

High School diploma or equivalent. Experience in administrative roles, preferably in healthcare. Strong organizational and multitasking skills. Excellent verbal and written communication skills. Proficiency in scheduling software and Microsoft Office/Google Suite. Understanding of care management principles. Responsibilities and Duties:

Manage and coordinate home care services scheduling. Collaborate with Care Coordinators and healthcare professionals. Use scheduling software for maintaining accurate calendars. Communicate scheduling changes effectively. Provide care management support, ensuring high-quality, person-centered care. Conduct assessments, develop care plans, and monitor client progress. Maintain compliance with regulations, care plans, and policies. Engage in training and professional development. Create and update personalized care plans. Collaborate with patients' families and social supports. Identify and recruit high-quality healthcare providers. Facilitate patient treatments and ensure treatment requirements are met. Suggest alternative treatment plans. Educate patients on procedures and referrals. Link patients to social services. Conduct regular follow-ups for improved health outcomes. Maintain case management records. Manage delivery and administration of health and personal care services. Create and maintain schedules for staff and clients. Manage information databases for staff and clients. Communicate schedule changes. Build professional relationships for excellent service delivery. Ensure accuracy in scheduling systems and maintain confidentiality. Requirements:

Proven experience as a Care Manager. Proficiency with Healthcare Management Systems and Microsoft Office. Outstanding communication skills. Excellent organizational skills. Ability to travel locally. Relevant Care Manager training/certifications (preferred). Strong interpersonal and multitasking skills. Good customer care ethics. Analytical and detail-oriented. Leadership qualities. Familiarity with database and payroll software. High school diploma: health-related post-secondary degree is advantageous. Minimum 2 years' experience in home care or customer service/healthcare management.

Pay Range:

$55,000 - $83,000 per year
