On Call every 5th weekend in the event of a staffing issue/concern.Rotational coverage of Holidays as well.Position Title: Neighborhood Nurse Manager FLSA Status: EXEMPTDepartment: Keystone CenterSupervisor Title: Director of Keystone CenterI. POSITION OBJECTIVEPromotes and maintains the elder's quality of life by developing day-to-day leadership and long-termplanning of the elder's clinical support needs; directs and develops licensed clinical care partners,collaborates with interdisciplinary team members; partners with Neighborhood Guide(s); providingphysical and psychological support for the elder's and their loved ones. Assumes 24-hour responsibility for the coordination of clinical activities in designated areas. Collaborates with all teams that support the elder's well-being and facilitates/fosters an elder-directed philosophy on assigned neighborhood. Supervises the nursing care of residents and the day-to-day nursing activities performed by licensed nurses on assigned neighborhood. Partners with the Neighborhood Guide to ensure that nursing assistants and life enrichment team members are supporting clinical aspects of the eider's plan of care. Such supervision must be in accordance with current federal, state and local standards, guidelines and regulations that govern the community and as may be required by the Director of Keystone Center to ensure that the highest quality care is maintained at all times. Functions as a nursing administration representative to physicians, ancillary team members, the elders, and families and/or significant others and the resident representative.Masonic Village at Burlington fully embraces a culture of hospitality. ESSENTIAL FUNCTIONS Assumes 24-hour responsibility for the coordination of clinical activities in designated areas. Directs the day-to-day (24 hours/day) care of the Licensed Nurses in accordance with current rules, regulations, and guidelines that govern the long-term care facility. Ensures that all assigned Team Members comply with established written policies and procedures. Is knowledgeable of the Quality Assurance and Performance Improvement (QAPI) Plan and promotes implementation and tean1 member education. Participates in stand-up meetings on assigned neighborhood. Communicates information to appropriate personnel in a timely manner. Facilitates a cooperative work environment and maintains collegial and supportive relationship with peers. Meets with licensed nursing team to plan and prioritize services to meet the needs of the elders. Seeks and accepts guidance from nursing administration in solving clinical and management problems. Conducts counseling sessions and administers corrective action as appropriate, providing the appropriate documentation to direct reports. Cooperates with other departments within the community when coordinating nursing services to ensure that the elder's total regimen of care is maintained. Consults with the elder's physician in providing care and treatment. Acts as a liaison between the elder, family, representative, physician, and other health care team members. Ensures that scheduled assessments and care plans are completed within the accepted time frames set by the MDS Coordinator. Ensures that a written plan of care (preliminary and comprehensive) is developed for each elder that identifies the concerns/goals of the elder, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care. Reviews documentation on care plans and medical records to validate standards are met. Ensures that all team members involved in providing care to the elders are aware of the elder's care plan. Makes daily rounds on the neighborhood to ensure that team members are performing their work assignments in accordance with acceptable nursing standards. Works flexible hours to include all three shifts if/as needed; provides weekend/holiday supervisory coverage as required. Reports and investigate all allegations of resident abuse and/or misappropriation of resident property. Participates in surveys conducted by authorized governmental agencies and/or consultants as may be requested. Fulfills the role of "charge nurse" for assigned neighborhood for a minimum of 5 days per pay period. Reviews complaints and grievances made or filed by team members. Makes appropriate reports to DON as required. Assists in the investigation and response of external complaints. Advises nursing administration of resident, clinical, licensed nursing personnel or elder problems/concerns. Holds regularly scheduled team meetings, including all three shifts, to promote continuity of care. Encourages active participation in planned changes, elder care issues, problem identification, and problem resolution. Retains minutes of meetings and informs Keystone Center Administration of relevant issues. Works closely with the rehabilitation team to ensure that the elders on assigned neighborhoods have care plans which address appropriate restorative therapy goals. Works in partnership with the Neighborhood Guide to ensure each elder lives life to the fullest on assigned neighborhood. Facilitates learning circles, fosters teamwork and growth of team members. Attend in-service and/or safety meetings as required.III. OTHER JOB FUNCTIONS Performs specific work duties and responsibilities as assigned by supervisor.IV. KNOWLEDGE / EXPERIENCE REQUIREMENTS Graduate of an accredited RN school of nursing required, BSN preferred. A minimum of five years' experience in a hospital or long-term care setting. Computer competency. Expertise in clinical are of practice. Administrative, management, and leadership skills. Superior customer service skills. Acts as a professional role model, innovator, and resources. Excellent written and verbal communication skills. Must be comfortable working with COVID-positive patients. Must be fully COVID and flu vaccinated per CDC guidelines and Masonic Village policy.V. LICENSES/ CERTIFICATIONS Licensed Registered Nurse CPR Certified VI.
