St. John's Episcopal Hospital is the only hospital providing emergency and ambulatory care to the densely populated, culturally and economically diverse, and medically underserved populations of the Rockaways and Five Towns in southern Queens County and southwestern Nassau County, New York. Celebrating over 110 years of community care, the 257-bed facility provides people of all faiths with comprehensive preventive, diagnostic treatment and rehabilitative services, regardless of ability to pay.

The Community Mental Health Center (CMHC) is a New York State Office of Mental Health licensed clinic providing therapy for individuals aged 5 and up. Our licensed social workers and psychiatrists offer psychotherapy, medications, and medication-assisted treatment. We offer various treatment approaches, such as Motivational Interviewing and Cognitive Behavioral Therapy. We also provide care for individuals with dual diagnoses of mental health and substance abuse disease. Our services are available in any language and can be accessed through a secure telehealth platform. We offer psychoeducation and help coordinate care for our patients. Status : Part-Time (37.5 hours biweekly)

Job Summary:
The Social Worker provides individual counseling sessions to patients on the Behavioral Health units as dictated by the treatment plan. Develops, implements, and conducts therapeutic groups relating to substance abuse, trauma, affective disorders, social skills enhancement, and medical illness on the inpatient BH units. Provides psychoeducation sessions for patients and their families/care partners.
Responsibilities: Completes psychosocial evaluations Completes collaborative treatment planning and implements patient centered services, involving family and /or care partners in the treatment plan process Conducts individual, group psychotherapy as prescribed by the treatment plan Participates in departmental meetings Documents according to clinical standards Develops after care plans for patients upon discharge including linkages to other agencies as appropriate Proficiency in implementing Suicide Risk Assessment Proficiency in implementing Telehealth Services Policy in compliance with Federal and NYS regulatory guidelines Required Skills:
Master of Social Work required Licensed in NY State LMSW or LCSW Minimum 2 years experience working in a Behavioral Health Outpatient Setting
