Job Description

Purpose:

The Reunion Program Coordinator is responsible for the implementation and creation of the Reunion Program activity calendar, recreational, socialization and validation programming for the Reunion Memory Care Program. The nonexempt Reunion Program Coordinator is primarily responsible for ensuring activity opportunities are available to memory care residents for socialization, engagement, exercise, community integration (as applicable to the resident's acuity level), spiritual development, recreation, and relaxation.

Scope & Status:

Non-Exempt Status/Hourly

The Reunion Program Coordinator reports directly to the Director of Memory Care. The Reunion Program Coordinator will also participate in the daily leadership team meetings and the Manager-On-Duty Program.

Relationships:

a. Engage with team members assigned to the Memory Care Community, encouraging them to participate in activities designed for resident engagement.

b. May also participate in marketing efforts, such as onsite tours and offsite presentations.

c. Interactions with residents, family members, support groups and referral sources

Qualifications:

To perform this job successfully, an individual must be able to perform and complete each essential duty satisfactorily, have excellent communication and language skills to enable them to perform their duties and interact effectively with residents, family members, guests, and referral sources. Team members must meet all specific state and federal regulations to assume the position at an assisted living community. In addition, the individual must have at least 1 year of experience working with the senior population, preferably with memory impaired residents and in a long-term care and/or health care setting. An on-going training program is required. A high school diploma and/or GED are required. The employee must have a working knowledge of MS Word, PowerPoint, and Publisher.

Employees must have and maintain a safe driving record. Reunion Program Coordinator models the values and core purpose of QSLM. The Reunion Program Coordinator utilizes specific skills involving art, crafts, design, imagination and innovative planning for activities and events. The Reunion Program Coordinator utilizes the company's technology and resources (specifically memory boxes, sensory stations, iN2L, engagement baskets) to effectively engage the residents.

Areas of Responsibility:

1. Planning

a. Successfully creates and implements the Reunion activity calendar daily. Monthly activity calendars should also tie into the QSLM annual/monthly theme.

b. Collaborates with the Director of Memory Care on activities and entertainment that best meet the needs of the residents.

2. Calendar Expectations

5-7 calendar activities per day, ensuring variety throughout the week, 7 days a week

• Daily exercise programs that stimulate both mind and body

• Minimum of one outing per week, for Memory Care, including shopping, dining out, and community experiences

• 2-3 spiritual offerings per week catering to diverse beliefs and preferences.

• Minimum of three lifelong learning opportunities and themed activities per week (e.g., cultural events, art and crafts, language classes, music programs, food-related activities)

• Weekly recreational and socialization opportunities for residents

• Two community-integrated opportunities per week (e.g., visits with children/students, outreach programs, volunteer involvement)

• On and off-campus volunteer opportunities for residents

• Outside entertainment at least once per week

• Minimum of three family events per month

• Host a monthly "A Taste of" themed meals coordinated with the dining department (e.g., special menu cards, specialty drinks, decorations, costumes). This should tie into the QSLM annual/monthly theme.

• Celebrations of major holidays and recognition of diverse religious preferences

3. Dates and deadlines

To assist with timely planning, QSLM has implemented the following dates & deadlines. If you are unsure how to schedule your calls or use teams for accessing them, speak to your Memory Care Director.

• 1st of the month- Following month's calendar template and ideas newsletter are released.

• 15th- Following month's calendar due to Memory Care Director and Executive Director for discussion.

• 20th - Following month's calendar draft due to Marketing for review.

• Monthly Activity Collaboration Call -

2nd Wednesday of Each Month

• Monthly Reunion Program Collaboration Call 3rd Wednesday of each month

• Monthly Community Social Activities Call (Scheduled by each community on a recurring monthly basis)

4. Direction and Implementation

a. Ensure residents have an opportunity to exercise each day. Receives instruction and recommendations from the Director of Memory Care.

b. Implements a resident walking/ambulation program according to the resident's needs and abilities.

c. Offers an adequate number of activities each day and ensures activities are available on Saturday and Sunday.

b. Assist with scheduling opportunities for residents to enjoy local cultural events according to the residents' service plan and ability to participate.

c. Utilizes the community passenger vehicle for transporting residents to and from community appointments, events, and scheduled activities. Provides direct assistance to residents while in the community.

d. Reports any changes in condition, hygiene/grooming issues and/or psycho-social concerns to the Director of Memory Care. Appropriately responds to residents' needs and concerns promptly.

e. Models' professionalism, compassion, and courtesy always.

a. Implement incentives for the residents to participate in all programs offered through the activities calendar. Maintains documentation on each resident's participation, scores, and progress.

b. Implements the Reunion Program for each resident at the time of admission. Assists new residents in adjusting to their unique environment and schedule. Ensures that new residents are placed at a dining table with other residents who share similar interests and socialization abilities.

c. Assists in the dining room during breakfast, lunch, and dinner to ensure residents have a positive experience.

d. Becomes familiar with the Reunion History Form for new residents, identifying interests, talents, hobbies, spiritual preferences, and social needs. Provides the new residents with the current activities calendar and promotes the memory boxes.

e. Assist in decorating and celebrating the major holidays, including but not limited to

Mother's Day

Father's Day

July 4th,

Veterans Day,

Memorial Day,

December Holidays,

New Years,

Resident Birthday

Valentine's Day.

f. Assist in ensuring that residents with different religious preferences enable people to celebrate their specific public holiday. Decorations are to be pre-approved by the Director of Memory Care and Executive Director.

g. Recommends innovative ideas to the Director of Memory Care for activities, socialization, spiritual development, and performance improvement.

h. Assist in making sure that the main activity multi-purpose room is neat, fun, and appealing to residents, family members, and guests touring the building. Utilizes other public areas effectively to maximize a variety in activities. Encourage resident participation in specific group and individual activities related to hobbies, talents and according to the resident's acuity level.

i. Assist in one-to-one visits with residents in their apartments and offers individual activities to those residents who may be unable to actively participate in a group setting.

j. Assist with personal invitations to residents to maximize participation in the activities program. Engages the help of personal assistants and nursing team members.

k. In the absence of the Director of Activities, the Reunion Program Coordinator may assist with activities for the residents on the assisted living side.

l. Assist with our bi-monthly family connections and email blast.

m. Captures photos of activities and events to showcase on social media. Ensures high- quality images are consistently submitted to marketing for the community's social media pages.

3. Regulation and Policy Compliance

a. Ensures that all QSLM policies and procedures are followed as it relates to activities, socialization, community integration, safety, and resident care.

a. Maintains Federal, State, County and City regulations. Follow all road and driving safety signs. Utilizes all equipment in a safe manner.

b. Completes the mandatory training course for operating the passenger vehicle and wheelchair lift. Immediately reports any maintenance needs to the Director of Maintenance and/or the Executive Director.
