Group Home Setting Patient Population:
Children with mental health and behavioral health challenges Responsibilities:
Conduct comprehensive assessments and care plans for individuals in the group home, gathering and documenting necessary information in compliance with regulatory guidelines-both in-person and remotely Perform intake assessments as part of the referral process, including reviewing records, conducting interviews, and making observations to identify service needs Actively participate in transitional planning and care coordination in collaboration with the Qualified Behavior Modification Professional (QBMP), including IEP, IPP, and IBSP meetings Identify and connect individuals with appropriate community resources to support their goals and well-being Qualifications:
Master's Degree in Social Work (MSW) Active registration or license to practice as a Social Worker in California Desired Skills:
Familiarity with California's Lanterman Act and the associated service delivery system Knowledge of Title 17 and/or Title 22 regulations
