The Opportunity:
Responsible and accountable for assessing the psychological social emotional cultural and or financial needs that impact patients health and recovery. To provide professional social work services that utilize supportive counseling and clinical knowledge in assessing the needs of patients and families during illness crisis intervention and post discharge planning. Collaborates with members of health care team in the development of a transition plan and addresses needs in a timely manner. Participate in program planning and development to meet the needs of customers within the health care system and community. Help ensure our vision to be,

Shift:
PRN, 3 shifts per month required, Sunday through Saturday, 8:00 a.m. to 4:30 p.m.
The Work:
Completes family therapy sessions with patients and their families.
Participates in daily treatment team meetings.
Provides psychoeducational group therapy sessions to patients.
Assists with discharging planning, making referrals and assisting with transportation arrangements
EXPERIENCE:
Prefer prior experience in a healthcare setting (i.e., hospital, home health or nursing home) but not required. SPECIAL SKILLS: Excellent communication, interpersonal, organizational and presentation skills. OTHER: Computer skills, Basic typing skills EDUCATION: Required:
Certified LMSW or LCSW
Missouri (Kansas) Social Work license
Master's Degree in Social Work
