Join Our Adult Inpatient Medicine, Surgery & Critical Care Teams Are you a forward-thinking clinician with a passion for innovation and complex care? We're seeking a skilled and dynamic professional to take on a new role as a supervisor in a collaborative project aimed at improving cross system communication, while caring for vulnerable and other medically complex patients.
In This Role, You Will:
Serve as liaison for collaborative project between acute and post-acute care facilities Conduct in-depth work to manage a caseload of complex ALOC and other patients to optimize their chances to transition to post-acute or community care. Carrie a caseload of complex patients. Act as a liaison and cultivates relationships with outside public and private agencies and post-acute care facilities. Provide clinical and administrative supervision of the social work staff including orienting new social work staff, continually apprising staff of changes in policy, procedure, or standards related to the department Ensure staff documents all interventions and continuing care plans in the patient's medical record in accordance with departmental and hospital policies. Participates in the department's performance improvement meetings and projects. Maintain and completes statistics and reports. Conduct and/or attend individual, unit, departmental and intra/inter hospital, outside agency meetings; attends case conferences Provide ongoing support and psychotherapeutic counseling to patients, families/significant others, as well as concrete services, and collaboration with interdisciplinary team Work hours are Monday-Friday, regular business hours. Ability to work from home one day per week after to be determined period as allowed by operational and clinical needs. Minimum Qualifications 1. Valid license as an LCSW issued by the NYSED and one (1) year of supervisory experience utilizing casework, group work and/or community organization methodologies. 2. Inpatient hospital experience preferred 3. Work hours are Monday-Friday, regular business hours. 4. Ability to work from home one day per week after to be determined period as allowed by operational and clinical needs.
