The salary range for this position is $60k - $75k annually and is commensurate with education, experience and hospice certifications. This is a full-time benefits eligible position. Location Primarily focused in Orange County, New York, but will be called upon, from time to time, to provide care for patients and their families in Rockland County, NY

Schedule: Monday through Friday, 8:30 AM to 4:30 PM. This includes a shared weekend rotation on a monthly basis

Responsibilities of Social Worker will include, though not be limited to the following: Contributes to patient and family counseling and education Participates in ongoing interdisciplinary comprehensive assessment Assesses patient/family psychosocial response to the terminal illness and evaluates coping skills Assists patients, family, and agency staff in utilizing community resources Assists team members in understanding the significant social and emotional factors related to the care of the terminally ill patient and family Coordinates with team members to plan and implement the hospice plan of care Social Worker will participate in interdisciplinary team meetings Social Worker maintains accurate, thorough and professional written records, and charts relevant information in a timely fashion, following UH documentation standards Participates in and is actively involved with the bereavement portion of the program as appropriate Participates in volunteer and staff education programs Develops and implements the discharge plan in conjunction with the IDS Participates in QAPI program and hospice sponsored in-service training Refers high risk patients Develops/maintains community resource file Incorporates organization's commitment to customer service and continuous quality improvement into daily work habits. This includes promoting positive co-worker and teamwork relationships and supporting the goals and mission as set forth by the agency Accepts additional responsibilities as assigned Qualifications:

MSW from a graduate school of social work is required. Must be licensed by the State of New York LMSW required, LCSW is a plus Bilingual English/Spanish is a plus Must have at least two years of supervised experience in the health field or two years of experience in family therapy/counseling Social Worker in this role must receive certification in hospice and palliative care within three years of their hire date and maintain certification throughout employment Ability to relate to a diverse population and workforce in a personable and effective manner Ability to relate to the terminally ill and their families. Bereavement experience preferred Must be proficient in MS Office Suite; experience documenting in an EMR is required Daily travel in a car is required to travel from patient to patient. Ability to drive with acceptable driving record for agency insurance coverage is required.

As a Social Worker with United Hospice, you will provide much-needed emotional support to patients and their families during these critical and challenging times. Your role will be essential in facilitating difficult conversations while addressing fears, hopes, and wishes with sensitivity and understanding, helping patients and their families through this journey. By working closely with our interdisciplinary care team, you will ensure that every patient and family receives the necessary resources to maintain comfort and well-being.
