PRIMARY PURPOSE
Duly's Value-Based Care organization is dedicated to transforming healthcare delivery by emphasizing quality outcomes and patient-centric approaches. Working closely with Clinical Review Nurses, the Clinical Project Manager is responsible for coordinating across multiple VBC clinical and operations teams, ensuring successful execution of patient interventions, and rigorously tracking performance metrics and outcomes. This position will play a key role in managing and improving the health of our most complex VBC patients.

Work cross-functionally across clinical and operations teams to coordinate patient care.
Work closely with team of Clinical Review Nurses to understand patient care needs Coordinate with appropriate VBC
and clinical
teams to drive and execute identified patient interventions Routinely lead cross-team discussions to track patient outcomes Showcase your data-driven, decision-making skills to measure and communicate the impact of change.
Rigorously track progress and execution of identified patient actions Utilize data analytics and insights to measure the progress and impact of identified
interventions. Assist in monitoring and measuring the effectiveness of changes and make recommendations to further attainment of goals Identify and surface opportunities to improve VBC care management programs based on clinical findings. Be a champion for cultivating a culture of change.
Build credibility with Duly stakeholders and handle conflict with professionalism and care Communicate impact findings effectively helping to foster a culture of adaptability, innovation, and continuous improvement Guide and support teams as they navigate through change Demonstrate your ability to successfully manage projects from inception to completion.
Develop and maintain project documentation, including charter, scope, goals, milestones, and deliverables Establish and meet timelines and due dates Communicate project objectives, goals, and progress to key stakeholders Partner with stakeholders to address potential risks Qualifications
Level of Education/Field of Study
Bachelor's degree in Business, Economics, Healthcare Administration or related field required Years of Experience
2-3+ years of previous experience in project management background required 1+ years of Healthcare experience preferred Describe Type of Experience
Proven track record of delivering operational results in a matrixed environment Background in in healthcare or value-based care Excellent communication and interpersonal skills with ability to influence stakeholders through clear, professional and confident presentations Advanced Microsoft office suite skills
The compensation for this role includes a base pay range of $85,000- $128,000, with the actual pay determined by factors such as skills, experience, education, certifications, geographic location, and internal equity. Additional compensation may be available through shift differentials, bonuses, and other incentives. Base pay is only a portion of the total rewards package.
